Job summary

Gentiva Hospice is seeking an experienced Registered Nurse to serve as an RN Case Manager, delivering skilled nursing care in patients' homes or other settings. You will assess, plan, implement, and document comprehensive care while coordinating with an interdisciplinary team to meet goals of care and support families through the end‑of‑life journey.

This role requires strong clinical judgment, communication, and the ability to educate and comfort patients and caregivers in sensitive situations.

Qualifications

• Registered Nurse with current license to practice in the employment state.
• 1+ year of RN experience in hospice, palliative care, home health, or related settings.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in‑person/telephonic RN assessments and provide education to patients, family members and caregivers as appropriate.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including patient visit frequency to meet individual needs and risk identification.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Provide patient and caregiver education regarding disease process, self‑care techniques, end‑of‑life care, medications, nutrition and dietary needs.
• Communicate change in condition to appropriate provider and ensure timely interventions.
