Job summary

Gentiva Hospice is seeking a dedicated RN Case Manager to lead in-home hospice care across patient and family support networks. You will assess, plan, and implement nursing care within individualized plans of care, collaborating with physicians, LPNs, social workers, and chaplains to ensure compassionate end-of-life care.

The role emphasizes patient and family education, pain and symptom management, and accurate documentation to maintain high-quality standards in home settings.

Qualifications

• Current license to practice nursing in the state of employment.
• Minimum of one (1) year RN experience in acute, hospice, or home health.
• Hospice or palliative care experience preferred.
• CPR Certification
• Current automobile insurance and valid driver's license.

Responsibilities

• Perform comprehensive patient assessments and reassessments, with education to patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across interdisciplinary teams including physicians.
• Educate patients and caregivers on disease, end-of-life care, medications, nutrition and self-care.
