Job summary

Qualifications

• Board certification in Internal Medicine or Geriatrics preferred.
• Experience leading interdisciplinary HBPC teams welcomed.

Responsibilities

• Provide patient care directly or via supervision of HBPC residents.
• Lead HBPC program development and coordinate with VA leadership.
• Perform assessments, diagnoses, treatments, and outcome measures.
• Oversee coverage planning and telehealth integration with in-person care.
• Collaborate with other HBPC physicians and VISN/VA staff.
