Job summary

The Minneapolis VA Medical Center, part of the U.S. Department of Veterans Affairs, seeks a full-time Physician for its Home Based Primary Care (HBPC) program.

You will provide in-home care, manage a patient panel, and collaborate with interdisciplinary teams to deliver comprehensive veteran care. Applicants should have an MD and be board certified/eligible in Internal Medicine, Family Practice, or Geriatrics; a pre-employment exam is required.

Qualifications

• Must hold or be eligible for board certification in Internal Medicine, Family Practice and/or Geriatrics
• Proficiency in medical documentation and electronic health records

Responsibilities

• Provide in-home primary care to a patient panel within HBPC program
• Coordinate care with interdisciplinary teams
• Deliver comprehensive care and manage chronic conditions
• Supervise and mentor medical trainees as assigned
• Work schedule typically Monday–Friday, 8:00am–4:30pm with weekend rotation
