The Minneapolis VA Medical Center is seeking a full-time Physician for its Home Based Primary Care (HBPC) program. This role provides care to veterans in their homes, coordinating with interdisciplinary teams and managing a patient panel with regular home visits and PACT responsibilities.

Candidates should have MD/DO with unrestricted US licensure, and be eligible for board certification in Internal Medicine, Family Practice, or Geriatrics. EDRP may apply; other VA total rewards are available.

Formación

• Proficiency in spoken and written English.
• Board certification/eligibility in Internal Medicine, Family Practice and/or Geriatrics preferred.
• Active, full, unrestricted medical license in the United States.

Responsabilidades

• Provide care to a panel of patients in Home Based Primary Care (HBPC).
• collaborate with interdisciplinary teams to deliver comprehensive care to Veterans at home.
• Manage in-person home visits and regular PACT duties.
• Engage in teaching and supervision of medical trainees as needed.
• Participate in the VA Education Debt Reduction Program (EDRP) process where eligible.
