Job summary

Optimal Care is seeking a Physician in Physician Services to deliver advanced primary and specialty care to geriatric and medically complex patients, often in patients' homes or care settings.

You will lead medical evaluations, develop treatment plans, and coordinate care with families and interdisciplinary teams. This role requires travel within the Main Service Area: Toledo and surrounding areas and offers a Monday–Friday schedule.

Qualifications

• MD/DO required.
• Current unrestricted State medical license required.
• Board certification in family practice, internal medicine, geriatric or palliative care.
• Prior supervisor/management experience required.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide advanced primary and specialty care to geriatric and medically complex patients, often in homes or care settings.
• Conduct comprehensive medical evaluations and manage acute/chronic conditions.
• Develop individualized treatment plans and coordinate care with families and interdisciplinary teams.
• Provide medical leadership and oversight for NPs and PAs, mentoring clinicians.
