Optimal Care - Evergreen in Carmel, IN seeks an experienced Physician Services clinician to provide advanced primary and specialty care to geriatric and medically complex patients, often in their homes, with autonomy and patient-centered focus.

You will conduct comprehensive evaluations, develop treatment plans, coordinate with families and interdisciplinary teams, mentor NPs and PAs, and help improve outcomes while reducing hospitalizations within a travel-enabled service area.

Qualifications

• Doctor of Medicine or Osteopathy with active license.
• Board certification in family practice, internal medicine, geriatric or palliative care.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide advanced primary and specialty care to geriatric and medically complex patients, often in homes or care settings.
• Conduct comprehensive medical evaluations, diagnose and manage conditions, and develop individualized treatment plans.
• Coordinate care with patients, families, and interdisciplinary teams; mentor NPs and PAs.
• Lead and improve patient outcomes, aiming to reduce hospitalizations and enhance quality of life.
