Optimal Care - Evergreen seeks a Physician Services clinician to deliver advanced primary and specialty care to geriatric and medically complex patients, often in their homes. You will lead and mentor Nurse Practitioners and Physician Assistants while coordinating with families and the healthcare team.

This role offers a collaborative, physician-owned environment with strong back-office support, flexible scheduling, and a focus on improving patient outcomes and quality of life in the Saginaw

Qualifications

• MD or DO with current unrestricted state license.
• Board certification in family practice, internal medicine, geriatric, or palliative care.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide advanced primary and specialty care to geriatric and medically complex patients.
• Deliver care in patients' homes or care settings with patient-centered approach.
• Coordinate care with patients, families, and interdisciplinary teams; mentor NPs and PAs.
• Improve patient outcomes and reduce hospitalizations through coordinated care.
