Optimal Care is seeking a Physician in Physician Services to deliver advanced primary and specialty care to geriatric and medically complex patients, often in patients' homes, with leadership over NPs and PAs. Expect time-focused, patient-centered care emphasizing comfort and function.

Travel within Toledo area and surrounding communities is required; home office in Maumee, OH. Competitive compensation and benefits supported by a clinician-owned team.

Qualifications

• Doctor of Medicine or Osteopathy
• Current unrestricted State license to practice medicine
• Board certification in family practice, internal medicine, geriatric or palliative care
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage

Responsibilities

• Provide advanced primary and specialty care to geriatric and medically complex patients, often in homes or care settings
• Conduct comprehensive medical evaluations and manage acute and chronic conditions
• Develop individualized treatment plans and coordinate care with patients, families and interdisciplinary teams
• Provide medical leadership and mentorship to Nurse Practitioners and Physician Assistants to ensure high-quality care
