Job summary
Optimal Care in Michigan is seeking a Physician Services clinician to deliver advanced primary and specialty care to geriatric and medically complex patients, often in their homes or care settings. You will lead medical care, evaluate conditions, develop treatment plans, and coordinate with families and interdisciplinary teams.
You’ll mentor Nurse Practitioners and Physician Assistants, help reduce hospitalizations, and improve quality of life while ensuring high-quality, patient-centered care
Qualifications
- MD/DO degree and board certification in geriatrics, family medicine, internal medicine or palliative care.
- Unrestricted state medical license to practice.
- Experience supervising staff and coordinating care teams.
- Reliable transportation with valid driver's license and insurance.
Responsibilities
- Perform comprehensive medical evaluations and manage acute and chronic conditions.
- Develop individualized treatment plans and coordinate care with patients, families and interdisciplinary teams.
- Provide medical leadership and oversight for Nurse Practitioners and Physician Assistants.
- Mentor clinicians to improve patient outcomes and care quality.
