Bloom Healthcare in Castle Rock, CO is seeking a Primary Care NP or PA to provide longitudinal, relationship-based care in patients' homes and communities. You’ll diagnose, manage chronic and acute conditions with support from an interdisciplinary team and advanced data systems.

This role offers a manageable schedule, 8-12 patients per day, light call, weekends off, a $5,000 sign-on bonus, comprehensive benefits from day one, and opportunities for bonuses based on performance.

Qualifications

• Master’s degree and active Colorado licensure as NP or PA.
• Experience with medically complex populations is a plus.
• Comfort delivering care in home and community settings.

Responsabilités

• Serve as the primary care provider for a defined patient panel, delivering longitudinal care in homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with interdisciplinary support.
• Address social determinants of health, including housing stability and access barriers.
• Conduct comprehensive evaluations and health education for patients and caregivers.
• Coordinate with care teams to reduce readmissions and ensure continuity of care.
• Utilize data systems to track outcomes and post-discharge follow-ups within 72 hours.
