Zusammenfassung

Centerstone is seeking a dedicated behavioral health professional to provide supports and services to clients and families in natural environments. You will deliver individual and group therapeutic interventions, assist with assessment and growth planning, and connect clients to services and natural supports to promote overall wellness.

You will collaborate with families, other providers, and the treatment team, documenting progress and advocating for clients while integrating physical health

Qualifikationen

• Bachelor’s degree or five years of paid mental health experience with a high school diploma may be considered in place of education.
• Current valid driver’s license and safe driving record are required.
• Strong communication, advocacy and collaboration abilities with clients and families.

Aufgaben

• Provide case management services including assessment, planning, linking, monitoring, referral and advocacy.
• Assist with WRAP development and wellness planning; conduct outreach and follow-up.
• Coordinate with primary care and other providers to integrate physical and mental health care.
• Document outreach attempts and ensure services follow the personal growth plan.
• Attend meetings, participate in committees, and report incidents per policy.
