Job summary

Centerstone in Illinois seeks a full-time clinician to provide client-centered supports in the client’s natural environment. You will deliver individual and group interventions, facilitate growth, build skills, and link clients to community resources and services to meet growth goals.

You will assist with assessment, planning, crisis response, and coordination with primary care and behavioral health teams, while advocating for clients and families and monitoring progress.

Qualifications

• Strong communication skills across written, verbal, in-person and virtual methods.
• Experience delivering client-centered supports and coordinating with families and providers.
• Knowledge of community resources and how to access them.
• Ability to document, monitor and advocate for service plans.

Responsibilities

• Develops and supports WRAP plans with clients and families.
• Provides outreach, advocacy, and follow-up per personal growth plan.
• Conducts ongoing case management including assessment, planning and linkage.
• Coordinates with primary care and other care providers to integrate physical and mental health care.
• Assists clients with transportation and building natural support networks.
