Qualifications

Responsibilities

• Provide group or individual trainings focusing on health and wellness.
• Develop and support WRAP plans with clients and families.
• Assist with navigating health care systems and barriers.
• Provide case management, referrals, and advocacy.
• Arrange transportation and follow-up services.
• Conduct intake and initial assessments.
• Coordinate with community resources and other providers.
