Qualifications

• High School Diploma or GED required; Associate degree in a healthcare-related field preferred.
• Experience in healthcare, public health, or patient services is preferred.
• Knowledge of navigation, care coordination, or case management principles preferred.
• Familiarity with programs like 340B, patient assistance, or grant-funded services preferred.
• Experience with data tracking, reporting, or program logs preferred.
• Strong communication, organizational, and problem-solving skills.

Responsibilities

• Provide patient-centered support for HIV prevention services, including PrEP education and adherence counseling.
• Monitor appointment adherence and laboratory requirements.
• Coordinate medication access and delivery through contracted pharmacy partners.
• Support patient recruitment and linkage to care with internal teams.
• Maintain accurate patient documentation in the EMR and program records.
