Job summary

South County Mental Health Center, Inc. in Delray Beach, FL seeks an HIV Early Intervention Specialist to provide HIV testing, prevention education, outreach, PrEP coordination, and patient support under the supervision of the Chief Medical Officer.

The role includes case management, EMR documentation, and collaboration with healthcare teams to link clients to medical and community services. Certification in Florida HIV/AIDS and phlebotomy is required; strong communication and office skills are

Qualifications

• Florida HIV/AIDS 500/501 Certification is required by Florida DOH.
• Minimum 1 year working in human services with outreach and case management for at-risk populations.
• Active Florida Phlebotomy Certification is required.
• Ability to work with diverse and vulnerable populations.
• Strong verbal and written communication skills.
• Proficiency with Microsoft Office is required.

Responsibilities

• Provide HIV testing, counseling, education, and referral services.
• Educate clients on HIV prevention, PrEP, testing, and treatment.
• Coordinate PrEP appointments and telehealth with the provider.
• Provide supportive case management and referrals to internal and external services.
• Document patient health history and HIV testing in the EMR.
• Collaborate with physicians, nurses, and community partners.
