Job summary

Via Care Community Health Center seeks an HIV Navigation Specialist to provide outreach, education, rapid HIV testing, and care coordination for individuals at risk for or living with HIV. You will connect clients to PrEP/PEP and HIV medical care, partner services, and supportive resources, while collaborating with East LA Women’s Center Promotoras and clinic staff to promote engagement and adherence.

Strong communication, cultural competence, and detail orientation are essential for success in

Qualifications

High School Diploma or GED required.
Active Basic 1 Rapid HIV Testing certification from LA County DHSP or willingness to attend training.
Experience in HIV education, outreach, or navigation preferred.
Spanish language skills encouraged.

Responsibilities

Conduct outreach and education with target populations in collaboration with partners.
Provide rapid HIV testing and confidential risk assessments.
Connect clients to PrEP/PEP and HIV medical care, ensuring adherence and follow-up.
Offer navigation assistance for HIV clinical care, treatment adherence, and case management.
Maintain records of activities and document program outcomes.
Attend meetings and coordinate with internal and external stakeholders.
