Job summary

Adelante Healthcare seeks an HIV/PrEP Navigator to engage patients and communities through outreach and care coordination for those living with or at risk for HIV. You will work with the clinical team to navigate care, address social determinants, and advocate for whole-person health.

Responsibilities include coordinating referrals, assisting with insurance navigation, prior authorizations, and supporting staff to meet program goals. Evening and weekend availability may be required.

Qualifications

• High school diploma or GED from an accredited institution.
• Graduate of an approved medical assistant training program.
• Two years or more in community outreach or clinical setting preferred.
• Bicultural experience with HIV/AIDS communities desired.
• English/Spanish interpretation certification required.

Responsibilities

• Coordinate and maintain outreach calendar and marketing material inventory.
• Promote HIV education, testing, prevention, and related services through community engagement and patient outreach.
• Support comprehensive patient care and case management within established timelines.
• Maintain patient confidentiality and HIPAA compliance.
• Accept program referrals and provide timely care coordination and navigation services.
