Job summary

Primary Care At Home, Inc is seeking a Social Work Case Manager to serve clients in Oakland with a hybrid work arrangement. The role focuses on assessing needs, developing care plans, coordinating services, and supporting clients living with HIV/AIDS through community-based care.

You will conduct home visits, manage case records, and collaborate with the care team to ensure timely access to housing, medical services, and psychosocial support.

Qualifications

• Bachelor degree or MSW from an accredited institution.
• Experience with social-service case-management and acute care for chronically ill clients.
• Strong verbal and written communication with reliable follow-through.
• Experience with communities of color, LGBTQI and HIV/AIDS.
• Bilingual capability in Spanish is a plus.

Responsibilities

• Conduct initial psycho-social assessments and verify eligibility for services.
• Maintain accurate records and case notes of all client interactions.
• Develop individualized care plans addressing medical, housing, mental-health and social needs.
• Coordinate eligibility, benefits, and resources with local, state, and federal agencies.
• Meet clients in their homes to provide psychosocial support and monitor progress.
• Participate in interdisciplinary meetings and unit trainings.
