Job summary

The Florida Department of Health seeks an HIV Case Manager to deliver medical case management across four counties. You will conduct comprehensive needs assessments, develop individualized care plans, and monitor client progress, coordinating linkages to medical, housing, and support services.

The role includes counseling, treatment adherence support, insurance navigation, and thorough documentation in CAREWare and HMS databases to ensure timely records and quality care for clients.

Qualifications

• Conduct comprehensive needs assessments and face-to-face client evaluations.
• Develop and monitor individualized care plans addressing medical, physical, and psychosocial needs.
• Maintain accurate case documentation and database entries in CAREWare and HMS systems within required timeframes.
• Coordinate client linkages to medical services, housing, substance abuse treatment, and government benefits programs.
• Monitor HIV medication therapy, treatment adherence, and lab values (CD4 and viral load).
• Provide client advocacy and navigation support for insurance, entitlements, and healthcare services.

Responsibilities
