Job summary

University of North Carolina at Chapel Hill seeks a Medical Case Manager (Social Work Practitioner) to provide case management, referral coordination, crisis intervention and eligibility determinations for adults with HIV receiving care at the outpatient Infectious Diseases Clinic.

You will conduct comprehensive assessments, identify patient needs, develop care plans, and facilitate access to medical, behavioral health and community resources, including food security, transportation and housing,

Qualifications

• Bachelor's degree in a Human Services discipline with coursework related to the area of assignment.
• Some positions may require certification to practice as a licensed Clinical Social Worker as required by the NC Social Work Certification Board.
• Experience in health care settings is preferred.

Responsibilities

• Provide medical case management, referral coordination, crisis intervention for adults with HIV.
• Identify patient needs, develop a plan of care, and facilitate access to medical, behavioral health and community resources.
• Coordinate with licensed social workers, medical providers, nursing staff, benefits coordinators, and community agencies to support patient engagement and health outcomes.
• Assist with food security, transportation, and housing needs.
