Job summary

Promethean Consulting Inc. seeks a Case Manager to join the HIV Clinical Care Team, manage a caseload, perform assessments, and submit encounter notes. The role covers Maryland’s 23 counties and Baltimore City.

You will facilitate access to intake and screening, develop comprehensive care plans, coordinate services, monitor medications, and maintain full credentialing and confidentiality in the electronic health record system.

Qualifications

• Active, unrestricted Maryland Nurse/Social Worker license.
• Associate’s degree in nursing or Master's Degree in Social Work preferred.
• Current CPR & First Aid Certification.
• Valid NPI and CAQH validation/re-validation.
• Access to private transportation.

Responsibilities

• Facilitate intake, screening, and assessment across settings.
• Co-develop comprehensive individualized Care Plans.
• Coordinate services and activities to implement Care Plans.
• Monitor medication side effects and lab results.
• Maintain confidentiality and licensure, and complete encounter notes within 24 hours.
