Zusammenfassung

PHMC is seeking a Medical Case Manager to coordinate care for adults with HIV/AIDS at the Care Clinic. You will report to the Health Center Administrator and manage approximately 50 active clients, coordinating with physicians, nurses, and patient navigators to ensure treatment adherence and timely documentation.

The role requires strong interpersonal and organizational skills, knowledge of community resources, and the ability to work with diverse populations.

Qualifikationen

• Bachelor’s degree in social work or related field from an accredited school of social work or equivalent.
• Associate’s degree in social work or related field, with intent to obtain a bachelor’s degree.
• Two years of HIV/AIDS case management or related work experience preferred.

Aufgaben

• Maintain a caseload of approximately 50 active clients.
• Act as liaison between clients and physician; advocate as appropriate.
• Complete AACO MCM assessment within 3 days of referral.
• Update MCM assessment annually.
• Develop a Medical Care Plan using standard templates.
• Provide treatment adherence and health literacy assessments at intake and annually.
• Conduct treatment adherence counseling at least quarterly.
• Identify barriers to medical care and refer to appropriate services.
• Face-to-face contact with a client at least every 90 days; ongoing phone contact.
• Attend medical appointments with each client at least every 90 days.
• Conduct one home visit per client annually.
• Maintain paperwork, flow sheets, signatures, and documentation.
• Coordinate with Care Team and Patient Navigators to locate those lost to care.
• Collaborate with an interdisciplinary team to evaluate patient needs.
• Maintain chart documentation for timely communication (CareWare 48h).
• Assess patient needs and refer to social services; report to grant systems.
• Enter and update CareWare data for monthly grant reporting (4800 units/year).
• Attend trainings; complete 20 hours CEU annually; provide proof to AACO Education Coordinator.
• Close cases when services no longer indicated or after 3 months of inactivity.
