• Certification to practice as a certified or licensed Clinical Social Worker may be required.

Responsibilities

• Provide medical case management, referral coordination, crisis intervention and eligibility determination for adults with HIV in outpatient Infectious Diseases Clinic.
• Assist with coordinating medical, behavioral health and community resources; support food security, transportation and housing needs.
• Connect patients to internal and community-based mental health and substance use services and financial benefit programs.
• Collaborate with licensed social workers, medical providers, nursing staff, benefits coordinators and community agencies to improve patient engagement and health outcomes.
• Attend meetings and participate in team-based activities as part of an interdisciplinary team.
