Job summary

GHR Healthcare is seeking a Nurse Practitioner to perform in-home health assessments across Lane County and select Oregon communities. This field-based role emphasizes preventive care, patient education, and coordination with primary care providers.

Candidates should have ANCC or AANP family/adult/geriatric certification, an active Oregon NP license, and proficient EMR skills. Flexible scheduling includes PRN or full-time options, with travel to clients’ homes and ability to lift up to 30 pounds.

Responsibilities

  • Conduct comprehensive in-home health assessments including history review and physical examination
  • Complete blood pressure, vital signs, and point-of-care testing as appropriate
  • Document diagnoses to support care management and treatment planning
  • Communicate assessment findings and care gaps to primary care providers
  • Educate patients and caregivers about disease processes and preventive care
  • Coordinate with primary care providers and community resources to close care gaps
  • Provide in-home visits with professional, safe, and accurate documentation

Nurse Practitioner - In-Home Health Assessments in Oregon - PRN or Full-time

Join our healthcare team as a compassionate Nurse Practitioner providing preventive, in-home health assessments across Lane County and select Oregon communities. Make a meaningful impact where patients feel most comfortable—in their own homes.

Nurse Practitioner Job Details
  • Location: Lane, Jackson, Marion, Multnomah, Polk, or Washington counties
  • Work Setting: Field-based, in-home visits and home health assessments
  • Employment Type: Local opportunity
  • Schedule: Completely flexible
  • Shift: 8-hour daytime shift scheduled within operating hours of 8:00 a.m. to 8:00 p.m., seven days per week
  • Duration: Approximately 52 weeks
  • Anticipated Start Date: November 2, 2026
  • Visit Volume: Approximately 6-7 visits during an 8- or 10-hour day, depending on patient needs and location
  • Role Focus: Preventive, non-prescribing health assessments, patient education, care-gap closure, and coordination with primary care providers
Nurse Practitioner Job Requirements
  • At least 1 year of clinical experience; family practice, adult, geriatric, or home health experience is preferred
  • Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • Active, unrestricted Nurse Practitioner license in Oregon or the ability to obtain Oregon licensure before the start date
  • Current national provider credentials and an unrestricted practice history without active disciplinary restrictions
  • Active, unrestricted driver's license and reliable transportation for in‑home patient visits
  • Proficiency with electronic medical records (EMR) and mobile technology
  • Ability to work independently in varied home environments and navigate stairs
  • Ability to sit, stand, kneel, and transport equipment weighing up to 30 pounds as needed
  • Strong communication skills and a patient-centered approach when working with older adults and Medicare populations
In-Home Nurse Practitioner Responsibilities
  • Conduct comprehensive in-home health assessments, including medical history review, medication reconciliation, vital signs, and physical examinations
  • Complete evidence-based health screenings and appropriate point-of-care testing
  • Identify and document diagnoses to support care management and treatment planning
  • Communicate assessment findings and care gaps to members' primary care providers
  • Recognize urgent or emergent conditions and take appropriate action or facilitate timely referrals
  • Educate patients and caregivers about disease processes, medications, preventive care, and treatment adherence
  • Address social determinants of health and connect patients with appropriate community and support services
  • Provide care directly in patients' homes while maintaining professionalism, safety, privacy, and accurate clinical documentation
  • Participate in training and orientation for assessment protocols, documentation standards, and point-of-care procedures
  • Coordinate availability and county coverage to support consistent scheduling and patient access
