Job summary

Advantmed, LLC is looking for a dedicated Nurse Practitioner (NP) to provide in-home wellness assessments across Oregon. In this full-time role, you will conduct health evaluations, perform medical examinations, and support chronic condition management. The ideal candidate will have an active NP license in Oregon and at least three years of direct patient care experience. Qualifications

• Minimum 3 years of direct patient care experience, preferably in primary care or with adults/geriatrics.
• Active, unencumbered Nurse Practitioner (NP) license in Oregon.
• EMR proficiency is desired.

Responsibilities

• Conduct comprehensive health evaluations in members’ homes.
• Perform medical history and physical examinations.
• Deliver preventive health education and counseling.
