Summit Home Care is a NYS licensed home care agency serving NYC and Long Island. We are seeking Home Health Aides (HHA) and Personal Care Assistants (PCA) to provide compassionate in-home care for adult and pediatric patients in Suffolk and surrounding areas.

Qualifications

• Must have New York State Home Health Aide Certificate.
• Government Issued ID, Social Security Card & Work Authorization, if needed
• 2 Reference Letters
• Recent Physical
• TB Screening (Quantiferon preferred)
• Rubella & Rubeola titer lab work

Responsibilities

• Daily personal care (bathing, grooming, oral care, dressing, etc.)
• Assist with the daily living activities per the Plan of Care
• Monitor patients and report on their condition
• Help with patient meals and feeding, as well as basic housekeeping functions
• Accompany the patient to doctor's visits, shopping, and other errands

Home Health Aide (HHA) Responsibilities:
