Job summary

North East Medical Services seeks a Health Services and Care Coordinator to support care coordination for patients in managed care programs. You will facilitate medical, behavioral, social, and community support services and develop multidisciplinary care plans with self-management goals and referrals.

Fluency in English required; Cantonese, Mandarin, Vietnamese, or Spanish helpful. The role includes in-person and phone coordination, data collection, and compliance with regulations.

Qualifications

• Strong English verbal and written communication skills with analytical and problem-solving abilities.
• Experience with customer service in healthcare settings preferred.
• Ability to manage multiple tasks and follow through with delegated responsibilities.
• Knowledge of healthcare compliance requirements a plus.

Responsibilities

• Be the system navigator and point of contact for patients enrolled in managed care programs.
• Provide enhanced care coordination services to help patients access care and overcome barriers.
• Track and support patients after ED or hospital discharges.
• Link patients with cultural and community resources for referrals and social needs.
• Develop and review care plans with clinical teams and support implementation.
