Qualifications

• High school diploma with 2+ years of relevant work experience.
• Experience with medical appointment scheduling; Epic EHR scheduling preferred.
• Experience in managed care settings a plus.
• Proficient with Microsoft Office and PC-based software.
• Strong interpersonal and communication skills.

Responsibilities

• Conduct outreach to newly assigned patients to establish primary care services and connect them with resources at NEMS sites.
• Conduct outreach to support population health initiatives, including wellness visits and preventive screenings.
• Engage diverse patient populations to promote NEMS services and community engagement.
• Identify needs and barriers to increase awareness and utilization of services.
• Assist with data collection and reporting of outreach activities to management.
• Track outcomes related to quality measures to support improvements.
• Participate in Quality Improvement activities and trainings as needed.
• Ensure compliance with all regulations and organizational policies.
• Perform other duties as required by manager.
