Job summary

The Care Navigator Coordinator position in Long Beach, CA is a non-clinical care coordination and patient advocacy role within AMM. You will act as a high-touch liaison for Medicare Advantage and value-based care patients, assisting with language access, appointment scheduling, transportation, and pharmacy support to ensure timely and coordinated care.

The role emphasizes cross-functional collaboration with PCPs, specialists, pharmacies, and health plans, with a focus on helping patients

Qualifications

• High school diploma or equivalent required; associate or bachelor's degree preferred.
• Bilingual fluency; Korean English required for Los Angeles / Orange County assignments.
• Ability to communicate effectively with senior/geriatric patients and caregivers.
• Strong customer service, organization, follow-through, problem-solving and multitasking skills.

Responsibilities

• Provide navigation, scheduling, pharmacy, benefit and care-coordination support.
• Coordinate specialist visits, imaging, labs, therapy, home health and related services.
• Track referrals and appointments; follow up on missed or delayed care.
• Coordinate transportation and connect patients with community resources.
• Document interactions and maintain patient confidentiality per HIPAA.
