Qualifications

• Bachelor's degree or LVN licensure and CA license required.
• Strong CPR certification and knowledge of chronic disease management.
• Excellent written and verbal communication; public speaking experience a plus.
• Ability to work independently and as part of a team; strong organizational skills.
• Willingness to travel to provide outreach presentations.

Responsibilities

• Act as primary contact for the family within a multi-disciplinary care team.
• Assist in development and implementation of individualized health care plans.
• Promote continuity of care and improve referral tracking.
• Support efforts to reduce diabetes-related complications.
