Job summary

Northwell Health seeks a Care Management professional to coordinate care for patients and families. You will use motivational interviewing to build rapport, support panel management, and address barriers to care in collaboration with primary and specialty teams.

The role emphasizes liaison work across hospitals, primary care, and community resources, with focus on high-risk patients and those recently discharged. Spanish language ability is a plus.

Qualifications

  • High School Diploma or equivalent required.
  • 1–3 years of relevant experience required.
  • Bachelor's or Master's degree strongly preferred.
  • Health home or care management experience strongly preferred.
  • NYS Driver's License strongly preferred.
  • Bilingual Spanish preferred.

Responsibilities

  • Uses patient-centered motivational interviewing techniques to build rapport and help patients improve their health.
  • Supports the primary care team by providing panel management to decrease lost to care and non-compliance.
  • Acts as liaison between hospitals, primary care providers, specialists and community resources for patients.
  • Identifies and tracks high-risk populations and preventive/chronic care needs.
  • Tracks patients discharged from inpatient or emergency departments.
  • Performs outreach and closes gaps in referrals and follow-ups across sites and communities.
