Job summary

Central Neighborhood Health Foundation is seeking an Enrollment Specialist to help patients enroll in health coverage and manage Medi-Cal renewals. You will provide outreach, document collection, and guidance on plan selections, working to minimize coverage gaps and ensure continuous access to care.

The role requires communication with patients, coordination with health plans, and proficiency with EHR systems. Training and certification in enrollment are provided as needed.

Qualifications

• High School Diploma or equivalent required.
• CAA (Certified Application Assistor) Certification required; if not held, must be obtained within six months of hire.
• One year of experience in a medical office or healthcare enrollment setting preferred.
• Bilingual English/Spanish skills preferred.

Responsibilities

• Provides enrollment and renewal assistance for Medi-Cal and CalFresh applications.
• Screens and educates patients on enrollment eligibility and plans.
• Enrolls patients in Covered California health insurance plans.
• Maintains accurate records in EHR and retention tracking systems.
• Conducts proactive outbound outreach to prevent coverage lapses.
• Assists with obtaining required documentation and communicating with health plans.
• Coordinates with DPSS and enrollment personnel to resolve issues and maintain coverage.
