Job summary

Central Neighborhood Health Foundation is a Federally Qualified Health Center dedicated to improving access to healthcare through enrollment support, renewal assistance, and community outreach. The Enrollment Specialist helps patients enroll in Medi-Cal and CalFresh, renew coverage, and navigate health plan options.

Responsibilities include proactive outreach, documentation in the EHR, and coordination with health plans to minimize coverage gaps, ensuring patients maintain access to services.

Qualifications

• High School Diploma or equivalent required.
• CAA (Certified Application Assistor) Certification required; obtain within six months if not currently held.
• Experience in healthcare enrollment, Medi-Cal eligibility, renewals, or related field preferred.

Responsibilities

• Assist patients with health insurance enrollment, Medi-Cal renewal, and retention.
• Conduct telephone outreach for renewal deadlines and retention.
• Enter and maintain patient information in the EHR system accurately.
• Coordinate with DPSS and health plans; connect patients with community resources.
• Prepare weekly productivity reports and maintain retention tracking.
