Central Neighborhood Health Foundation is seeking an Enrollment Specialist to help patients enroll in health insurance plans, manage Medi-Cal renewals, and assist with CalFresh applications. You will educate patients on plan options, complete required documentation, and maintain accurate data in the EHR to reduce coverage lapses.

The role emphasizes proactive outreach, coordination with health plans and social services, and a focus on timely renewals.

Formación

• Must be able to communicate effectively with patients, medical staff, and families.
• Knowledge of Electronic Health Record (EHR) systems and accurate data entry.
• Familiarity with health insurance programs such as Medi-Cal and Covered California.
• Bilingual English/Spanish skills preferred.
• Willingness to obtain CAA certification within six months if not already certified.

Responsabilidades

• Assists patients with health insurance enrollment and Medi-Cal renewal.
• Guides patients through CalFresh applications and renewals.
• Performs proactive outreach to patients at risk of losing coverage and supports renewal documentation.
• Maintains patient information in the EHR and tracks enrollment outcomes.
• Coordinates with DPSS, health plans, and enrollment staff to resolve eligibility issues.
