Job summary

Easterseals Midwest is seeking a Health Home Coordinator to gather and process information for Health Home enrollments and transfers, while ensuring data accuracy and compliance with guidelines.

You will monitor hospital admissions/discharges, extract screening data, and generate reports for staff. The role requires collaboration in a team environment and strict adherence to HIPAA regulations.

Qualifications

• High school diploma or equivalent required.
• Bachelor's degree in human services or related field preferred.
• One year of office/administrative experience required; healthcare setting preferred.
• Experience using Microsoft Office suite and intermediate Excel skills required.
• Previous experience with Electronic Health Record (EHR) systems preferred.

Responsibilities

• Gather, research and process Health Home program information for enrollments, transfers and discharges.
• Enter and verify data in DMH and internal systems within established timeframes.
• Monitor hospital data portals and communicate admissions/discharges to nursing staff daily.
• Extract metabolic screening values from records and input into DMH and internal systems.
• Run scheduled and ad-hoc reports to support program goals and decisions.
• Maintain multiple tracking systems and ensure data accuracy and timeliness.
• Identify high utilizers of emergency care for reporting purposes and support outreach.
