Job summary

Compassus is seeking a Hospice Team Coordinator to manage patient data, admissions, and records within the HCHB and EMR systems. The role coordinates among the interdisciplinary team and ensures accurate billing and timely documentation for quality patient care.

The position requires strong communication skills, attention to detail, and experience with medical records or office support. A high school diploma is required.

Qualifications

• High school diploma or GED required.
• Experience with medical records, medical office, or business office support preferred.
• Ability to read, write, and interpret general business materials.

Responsibilities

• Accurately enters and manages patient data, including referrals and admissions.
• Ensures admission documents are complete and signed.
• Uses HCHB to maintain patient records and generate compliance reports.
• Monitors DME orders and informs management of trends.
• Coordinates IDT activities and serves as central contact for hospice team.
• Handles calls and maintains a welcoming atmosphere.
