Allegheny Health Network is seeking a proactive health team member to assist in population health management, coordinate care, and support providers during visits. You will perform chart reviews, engage patients with motivational interviewing, and help with data entry and order entry under supervision.

The role emphasizes teamwork, adherence to protocols, and maintaining inventories and sterile technique to ensure smooth office operations and quality patient care.

Qualifications

• High School / GED or completion of a Medical Assistant Program or LPN license.
• Current PA LPN licensure or multi-state licensure via eNLC or Medical Assistant certification.
• 2 years of Medical Assistant experience.

Responsibilities

• Participates in population health outreach and closes care gaps using registries and analytics.
• Completes pre-visit planning with chart and data reviews before patient visits.
• Assists provider during visits and activates standing orders for care gaps.
• Documents clinical encounter elements under supervision and assists with order entry.
• Coaches patients using motivational interviewing to support self-management.
• Performs data and registry reviews to monitor population health goals.
• Identifies high-risk patients and coordinates complex care referrals with the team.
• Reviews access and care flow to reduce avoidable ED visits.
• Prepares and sterilizes instruments, manages supplies, and maintains treatment areas.
• Performs in-house testing in accordance with policy and standards.
• Performs other duties as assigned.
