Qualifications

• 2 years Medical Assistant experience preferred.
• CPR certification required.

Responsibilities

• Participates in population health outreach to identify gaps in care using analytic tools and patient registries.
• Assists in pre-visit planning by reviewing charts and data.
• Supports patients and providers during visits and activates standing orders as appropriate.
• Documents clinical encounters under supervision and assists with order entry.
• Uses motivational interviewing to coach patients toward self-management and activation.
• Performs data review to support population health goals.
