Allegheny Health Network is seeking a healthcare team member to assist with population health management through patient outreach and care coordination. You will support clinical visits, provide self-management coaching, and help maintain efficient office operations.

The role emphasizes data-driven care, adherence to protocols, and leadership within care teams to improve patient outcomes. On-site responsibilities include instrument sterilization and inventory management.

Qualifikationen

• High School diploma or GED; completion of Medical Assistant program or LPN license.
• Minimum 2 years of Medical Assistant experience.
• Current PA LPN license or multi-state licensure via eNLC or Medical Assistant Certification.

Aufgaben

• Outreach to identify patients with gaps in care using analytic tools and registries.
• Perform pre-visit planning by chart/data review and prepare for patient arrival.
• Assist provider in addressing care gaps and activate standing orders.
• Document clinical encounters under supervision and assist with order entry.
• Coach patients using motivational interviewing to support self-management and activation.
• Review registries and conduct population outreach to close gaps in care.
• Identify high-risk patients and support complex care management referrals.
• Improve patient access and reduce avoidable ER visits.
• Sterilize instruments and ensure adequate inventory of supplies.
• Perform in-house testing following established standards.
• Perform other duties as assigned.
