Job summary

Imagine the Possibilities, Inc. in Iowa is seeking HCBS Programmers for an in-office role within Home and Community-Based Services. You’ll monitor services, develop individualized plans, coordinate with CBCM and IDT, and lead a team to empower people to meet their goals.

Responsibilities include building partnerships, maintaining accurate documentation, and ensuring compliance while advocating for independence and dignity. The schedule is Monday–Friday, 8:00 a.m.

• Develop individualized service plans with CBCM/IDT
• Communicate service changes to families, guardians, and case managers
• Ensure compliance, accurate incident reporting, and documentation
