Job summary

Imagine the Possibilities, Inc. seeks Direct Support Professionals to provide person-centered care in Home and Community-Based Services settings across Iowa.

You will support individuals to reach their goals, implement behavioral care plans, document accurately, and collaborate with a care team to promote independence and community participation.

The role requires compassionate communication, adherence to Iowa Code, and flexibility with shifts to meet program needs.

Qualifications

• High school diploma or equivalent required.
• Must be able to work in the U.S. without sponsorship.
• Driver’s license required for transportation duties.
• Excellent communication and interpersonal skills.

Responsibilities

• Develop individualized goals and provide direct support focusing on skills, independence, and community participation.
• Use behavioral strategies to support individuals while following rights restrictions.
• Complete documentation in Setworks daily, ensuring accuracy and compliance with Iowa Code.
• Provide hands-on support in home and community settings, including transportation and safety monitoring.
• Maintain attendance and complete training; communicate concerns through appropriate channels.
