Job summary

Imagine the Possibilities, Inc. is seeking an HCBS Coordinator to lead a team providing community-based services under HCBS settings in Iowa. The role requires in-office presence, a Monday–Friday schedule from 8:00 a.m.

to 4:30 p.m., and participation in an on-call rotation with flexibility for open site hours as needed. You will monitor services, develop strategies to help individuals achieve goals, maintain records, and ensure compliance with governing rules.

Qualifications

• Confident advocate who knows when to lead and when to follow.
• Must be at least 18 years old.
• Must have a valid driver’s license and reliable transportation.

Responsibilities

• Staff and supervise Direct Support staff, manage training and on-call coverage.
• Ensure HCBS compliance, policies, and DHS regulations; conduct site visits.
• Develop, implement, and monitor Individual Service Plans with case managers and IDT.
• Oversee medication administration, appointments, and budgeting/payee responsibilities.
• Serve as primary liaison with case managers, families, and stakeholders; manage incident reports and transportation coordination.
