Job summary

Imagine the Possibilities, Inc. operates in an HCBS setting, delivering medical, social, and supportive services to Iowans with health needs. The role emphasizes leadership, coordination, and compliant documentation within a team that supports individuals to achieve personal goals.

As a core team member, you’ll monitor services, develop individualized plans, and coordinate with CBCM and IDT. This full-time position runs Monday–Friday with 8:00 a.m.–4:30 p.m. shifts and an on-call rotation.

Qualifications

• Must be at least 18 years old and able to work in the U.S. without sponsorship.

Responsibilities

• Lead and develop a cohesive team culture with active presence and coaching.
• Oversee resources, budgets, and scheduling to ensure smooth service delivery.
• Coordinate person-centered plans, assessments, and documentation with care teams.
• Serve as a primary liaison with families, guardians, and case managers while ensuring compliance.
