Job summary

Imagine the Possibilities, Inc. is seeking an HCBS Coordinator to lead a team delivering community-based services in Iowa. The role emphasizes individual choice, coordination of services, and compliance with DHS rules, with a focus on empowering people to achieve goals.

You’ll monitor case plans, maintain records, supervise Direct Support staff, and collaborate with families and providers to ensure quality care while maintaining necessary on-call coverage and a Monday–Friday schedule.

Qualifications

• Age 18+ with a valid driver’s license and reliable transport.
• Proof of automobile insurance required.

Responsibilities

• Staffing and supervision: hire, schedule, train, evaluate, and ensure coverage including on-call.
• Quality and compliance: ensure HCBS policies, DHS rules, and conduct site visits.
• Coordinate and monitor Individual Service Plans with case managers and IDT.
• Oversee medication processes, scheduling follow-ups, and finances.
• Liaise with families and stakeholders; manage reports, transportation, and incidents.
