JAG Physical Therapy is seeking a licensed Physical Therapist in New Jersey for a patient‑centered, outcomes‑driven role. You will assess patients, design creative treatment plans, and guide a high‑energy team to maximize functional independence.

Qualifications

• Bachelor’s or Master’s Degree required; Doctorate preferred.
• Licensed Physical Therapist in New Jersey.
• Completed state CEU requirement (NY, NJ).
• Valid CPR certification.
• Experience with EMR software preferred.
• Excellent oral and written communication.
• Strong clinical decision‑making and teamwork.

Responsibilities

• Collect and record patient care data per guidelines and regulations.
• Perform initial evaluation, daily notes, and progress reports.
• Develop and adjust plans of care with interdisciplinary team.
• Deliver musculoskeletal interventions: therapeutic exercise, mobilization, modalities.
• Educate patients and caregivers; document outcomes in EMR.
• Oversee PT aides and participate in meetings and community outreach.
