Job summary

JAG Physical Therapy is seeking a dedicated Physical Therapist to join its forward-thinking team. You will assess patients, develop creative treatment plans, and support healing and functional independence in a high-energy environment.

Qualifications

• Bachelor’s or Master’s Degree required, Doctorate degree preferred
• Licensed Physical Therapist in the state practicing in
• Completed state regulated CEU requirement in appropriate state (NY, NJ)
• Valid CPR License
• Experience/knowledge with an EMR software is preferred
• Excellent oral, written communication
• Strong clinical decision-making skills
• Excellent work ethic and dedication to patient success
• Basic computer skills
• Works as a team member and individually with minimal supervision
• Maintains a positive, professional demeanor at all times

Responsibilities

• Collects and records patient care data following guidelines of the patient care service and third parties
• Includes initial evaluation, daily notes, progress reports, discharge plans and team communication
• Records daily billing and follows productivity guidelines
• Prepares for the evaluation per department policies and standards
• Selects and administers tests and measures
• Synthesizes evaluation results to form diagnosis, goals, prognosis and plan of care
• Establishes treatment plan and collaborates with the interdisciplinary team and patient
• Identifies need for consultations or referrals and initiates discharge plan
• Provides education/training to patient and support system
• Training includes ADLs/IADLs, transfers, rehab equipment training and community reintegration
• Responsible for treatment and exercise prescription for caseload
• Delivers safe, effective interventions in line with JAG standards
• Musculoskeletal interventions including therapeutic exercise, joint mobilization and modalities
• Note writing using Prompt EMR for all treatments
• Oversees PTA, ATC, and PT aides
• Attends annual JAG Billing and Coding seminar
• Promotes JAG in the community through lectures and presentations
