JAG Physical Therapy in New Jersey is seeking a licensed Physical Therapist to assess patients, craft personalized treatment plans, and help patients regain function and reduce pain. The role emphasizes accuracy, discretion, and a positive environment.

You will evaluate, treat, and document daily progress while supporting patient confidentiality. Requirements include NJ licensure and a degree in physical therapy (DPT preferred); EMR experience is a plus.

Qualifications

• Licensed Physical Therapist in NJ
• Completed state CEU requirements (NY, NJ)
• Strong communication skills (oral/written)
• Ability to work in a team and independently with minimal supervision
• Proficient with EMR software

Responsibilities

• Conduct initial evaluations and daily notes.
• Develop and adjust treatment plans and goals.
• Collaborate with interdisciplinary team for patient care.
• Document care and billing in EMR per guidelines.
• Provide patient education and training for ADLs/IADLs.
