Job summary

JAG Physical Therapy in New Jersey is seeking a licensed Physical Therapist to assess patients, design and implement treatment plans, and monitor progress in a high-energy outpatient setting.

You will collaborate with an interdisciplinary team, maintain patient confidentiality, use EMR tools, and contribute to achieving functional outcomes. The role offers a strong sign-on and relocation package, annual reviews, and opportunities for growth within a supportive environment.

Qualifications

• Bachelor’s or Master’s Degree required; Doctorate degree preferred.
• Licensed Physical Therapist in the state practicing in
• Completed state regulated CEU requirement in appropriate state (NY, NJ)
• Experience/knowledge with an EMR software is preferred
• Excellent oral, written communication and strong clinical decision-making

Responsibilities

• Collects and records patient care data per guidelines and reimbursement systems.
• Perform initial evaluation, daily notes, reassessments and discharge planning.
• Prepare for evaluation per department policies and standards of practice.
• Establish treatment plan based on evaluation and best practice; collaborate with team.
• Identify need for consultations and refer to other health care professionals.
• Provide education/training to patient and support system.
• Oversee PT aides and staff as needed.
• Promote JAG in the community through lectures/presentations
