JAG Physical Therapy in New Jersey is seeking a licensed Physical Therapist to join our care-first outpatient team. You will assess patients, design effective treatment plans and provide hands-on care in a high-energy, supportive environment.

Qualifications

• Bachelor’s or Master’s Degree required; Doctorate degree preferred.
• Licensed Physical Therapist in the state practicing in NJ.
• Completed state regulated CEU requirement in NY/NJ.
• Experience with EMR software preferred.
• Excellent oral, written communication; strong clinical decision‑making.

Responsibilities

• Collects and records patient care data per guidelines, including initial evaluation and progress notes.
• Records daily billing and follows department productivity guidelines.
• Establishes treatment plans and collaborates with the interdisciplinary team.
• Provides patient education and training for ADLs/IADLs and rehabilitative activities.
